PCP for insurances
New Horizons Primary Care is an open access facility using extenders for primary care.
At present we can be your Primary Care Provider (PCP) for Medicare, Medicaid, Anthem, Aetna, BCBS, Humana and United Healthcare.
We also accept self pay.
We see patients aged sixteen and above.
Our panel for patients with Medicaid as secondary is now full.

Conditions We Treat
Providers and medical office team at New Horizons Primary Care provide affordable, accessible and complete primary care to patients of Denver and Aurora Metro Area. Since 2013, we offer a full scope of medical services, including new patient and follow up visits, yearly physicals, annual vaccinations, management of acute and chronic medical conditions such as diabetes, hypertension and heart disease, thyroid and endocrine conditions, COPD and asthma, or anxiety and depression. EKG, spirometry, swabs, HB A1C and INR testing is readily available and finger stick or on site labs can be drawn during the appointments.
The providers routinely screen patients for a wide range of chronic conditions, including high cholesterol, early heart disease, diabetes, sexually transmitted infections, digestive diseases, attention deficit disorder, insomnia and more. Flu, shingles, pneumonia, tetanus, hepatitis and other vaccines are also available. Preventive medicine is a high priority for New Horizons Primary Care, to catch signs and symptoms of chronic diseases and help patients feel better. We care deeply about our patients and their families, value their lifestyle and time and understand the economic and social impacts of the illnesses. Appointments
The best way to book appointment is to "book online". After booking appointment, please check your phone, email for check-in instructions. We want all our patients to be checked in before their appointment time. This saves you time. It also reduces the chance of errors and allows targeted exams in the time stipulated. We have built a work flow for self check in and self check out. This reduces the errors. It is really important for you to use self check in and self check out.
We also limit the number of patients seen by extenders during their shifts. Extenders are asked to spend half an hour time on each patient by restricting total visits for the day to only sixteen visits. Their time includes the time they spend calling labs, reviewing results, sending prescriptions, writing and reviewing referrals. So please be organized. If you believe something got missed provide that information while reviewing the visit at checkout.
